MEDICARE CHRONIC DISEASE PROGRAM
A Chronic Disease Management (CDM) referral allows your GP to plan and coordinate your health care with an Allied Health Professional such as an Exercise Physiologist. At Restart, we help people in pain suffering with limitations and diminished quality of life ditch anxiety and regain confidence in their body by creating effective, personalised rehabilitation plans.
